Please note if you have had any serious reactions to make up before or have severe allergies to cosmetics I may be unable to do your make up - please speak to me before completing this form if you're unsure. Please be aware lash glue contains latex so if you have any issues with latex please inform me.

Are you happy for me to share images of your hair and make up online from our appointments?

I love to take photos for my portfolio to share on my social media and website. Please confirm if you are happy for me to do this. If you change your mind at any time you are welcome to inform me!

*Make up Notice* - please note I must be made aware prior to an appointment if you have a coldsore or any infectious conditions as this will affect the service. I politely request if you are unwell to reschedule where possible. Thank you for your understanding.
